1. ** ALL PACKAGES MUST BE COMPLETED BEFORE THE TERM ENDS.THEY MAY NOT BE CARRIED OVER UNLESS DUE TO ILLNESS, COVID PROTOCOLS OR SERIOUS INJURY.
2. The parents are obligated to inform the company whenever the player develops any new medical conditions in the future.
3. The player shall commit to abide by the rules of conduct set by Hoop Mountain Sports Services LLC at all times, the company (Hoop Mountain Sports Services LLC) has the right to terminate his/her membership without refund if stated rules are broken.
4. The player commits to exclusively wearing the official uniform (shorts and jersey) provided, upon paying the cost.
5. In the event a player is injured, the parents do not have the right to request any financial compensation from the company (Hoop Mountain Sports Services LLC) nor the school that owns the gym. In the event of an injury the company (Hoop Mountain Sports Services LLC) reserves the right to request immediate medical services.
6. In the event the player(s) cancels his/her membership before the date of expiration, the player will not be refunded.

Parent Consent:

1. I hereby acknowledge and confirm, I have read all the terms and conditions included in this registration form and I will cover all the required expenses.
2. I acknowledge that the specified player(s) declared are medically fit to participate in all physical activities.
